Behavioral Sleep Problems in Children & Adolescents

Difficulty falling asleep, night wakings, dependency on a parent, solutions for infants, toddlers and adolescents

Behavioral sleep problems are among the most common causes of sleep difficulties in childhood. They include difficulty falling asleep alone, the need for a parent's presence, bedtime battles, bedtime resistance, and repeated awakenings that require outside help to fall back asleep. In sleep medicine literature they are classified as behavioral insomnia of childhood, mainly the sleep-onset association type and the limit-setting type.

In early childhood, the problem is often linked to a strong association between falling asleep and an external condition, a 'transitional object': this can be a parent in the room, a bottle, rocking, touch, essentially anything that happens during the half hour before independent sleep onset. When the child wakes naturally at night, they struggle to fall back asleep without those conditions, and in their absence may also show signs of anxiety and a sense of abandonment.

In adolescence, sleep problems tend to be different: bedtime procrastination, screens, difficulty disconnecting from stimulation, a gap between the natural sleep time and school demands, and sometimes anxiety or emotional load.

Treatment is based mainly on behavioral intervention: a consistent bedtime routine, clear limits, independent sleep onset, positive reinforcement, and aligning expectations with the family. In children and adolescents with co-existing conditions such as ADHD, ASD or anxiety, the plan must be personalized.
